MEMBERSHIP CARD
A membership card is issued to a Chamber member listed in the directory and register of medical doctors.
Members must report the loss of their membership card to the Chamber without delay.
Members may request a duplicate membership card if the original is lost, stolen, damaged or destroyed.
An application for a duplicate or replacement membership card must be submitted using the prescribed form.
Duplicate and replacement membership cards are issued in accordance with the Rulebook on duplicates and replacements of licences / temporary licences to practise, facsimile stamps and membership cards.
A new membership card is issued within 30 (thirty) days of the application being submitted.
